ASC Form and Application

The Westminster After School Care Program (ASC) is licensed by the Department of Early Care and Learning. The program is open to children enrolled at Westminster Day School, PreK to 5th grade. The daily schedule begins at noon with lunch, free play, and naptime for the PreK and Kindergarten classes. 1st through 5th graders come to ASC upon release from school. An opportunity to complete daily homework assignments and participate in arts and crafts projects is available to each class. ASC provides a nutritious snack to all children in the program between 2:30 and 3:30 pm each day.

This program schedule is open each school day from noon - 5:30 pm, with the following exceptions for the 2006-2007 school year:

October 13th	Friday	Faculty Work Day
October 26th	Thursday	Parent-Teacher Conferences
November 21st	Tuesday	Day Prior to Thanksgiving Holidays
December 15th	Friday	Noon Dismissal for Christmas Holidays
January 2nd	Tuesday	Faculty Work Day
January 15th	Monday	Martin Luther King Day
February 19th-23rd	Monday-Friday	Mid Winter Holidays
March 9th	Friday	Faculty Work Day
April 2nd - 6th	Monday-Friday	Masters Week
May 18th	Friday	Field Day
June 1st	Friday	Early Dismissal/Last Day of School

In order for your child to attend ASC this year, the Enrollment/Parental Agreement Form and the Medical Information/Emergency Authorization Form must be on file and signed by both parents/guardians.

The hours and fees for enrolled students are as follows:

1 hour	$5	4 hours	$14
1 1/2 hours	$6.50	4 1/2 hours	$15.50
2 hours	$8	5 hours	$17
2 1/2 hours	$9.50	5 1/2 hours	$18.50
3 hours	$11

Maximum charge for noon - 5:30 pm attendance: $60 per week

Maximum charge for 3:10 pm - 5:30 pm attendance: $50 per week
